Health History Update

Should we be making patients fill out a new health history if it has been over 4 years old or just initial if everything has stayed the same? The front desk and I are not on the same page, any insight would be helpful!

I believe the ADA recommends that we do a completely new medical history every two to three years, depending on the health status of our patient. Beyond that, medical history should be reviewed at every appointment. I hope that helps!

I check health history every visit and the front office has them do a new HHx every year.

Awesome, some of our patients get mad at us if their HH hasn’t changed and we make them fill out a new one if its been a couple years. Was just curious what the protocol should be. Thanks again!

we do a complete one every 2 years

We have an update sheet that is filled out at every visit. They are asked to list health conditions, medications, and allergies.

Our form has an area on the back for 4 updates at 6 mo that is two years, at which we have them fill out a new one.
