FMD vs SRP

Just came across someone’s FMD question, so now have a question of my own. Is a full mouth debridement considered the first step to do SRPs or are you just suppose to get straight into SRPs? I’ve noticed my Dr will tx plan a full mouth debridement and tells us to tell the patient that it’s the first step for a deep cleaning. Most agree to do it. Now shadowing RDHs, I’ve noticed they get straight to SRPs. Am I missing something?

Technically, a FMD should only be used if the patient has so much material alba, plaque, calculus, etc. that a doctor cannot do a proper exam. It is supragingival. However, with FMDs there is a chance that if the gingival tissue heals around calculus a perio abscess can result. Which is why I only do them if absolutely necessary. I would rather go straight to SRPs. I find a lot of dentists treatment plan FMDs to squeeze more money out of a patient. I don’t think you are missing something!!

I agree with Kara. I know some office use this protocol to make more production. A FMD isn’t a high priced procedure so it’s not helping production that much. I think it’s okay to use this on a gingivitis pt that will come back in 2-6 weeks for an 1110 to fine scale and make sure you remove what was missed during FMD due to bleeding etc…
