Patient treatment journal

My doc wants me to start keeping a journal on patients. Is anyone doing this? Or have you in the past? What exactly are you keeping track of and is it beneficial? Tips and thoughts appreciated.

I have never done this (or even been asked to do this) because this is what chart notes are for. I have however kept personal notes about a patient (kids, grand kids, recent vacations, etc.) in a separate note section outside of the patient’s chart notes. It takes enough time to write in chart notes, let alone write a separate journal entry about a patient, so I’m not sure this is an efficient use of your time. I wonder where your doctor heard about doing this?
