Perio Protocol?

I am the only hygienist at my office and I work “accelerated” hygiene with an assistant. I feel that the hygienist before me did not implement any perio protocols/programs. I look at the notes and occasionally find perio charting. I have been recording probing depths on all new patients since I have started, and spot probing all others.

I might also mention that the dentist prefers 4BWX and 6 anterior PA’s taken every 6 months AND pano 1 x year, so this takes time away from me! I requested to adjust the schedule to allow for me to perio chart, and so that I’m not jumping from room to room (they agreed I was over-booked).

I’m a new graduate, so I’m not familiar with perio protocols besides 3 hour appointments at school. Ideally, I would like to do a complete charting on new patients and then yearly there after. But like I said, it’s hard when radiographs are taken at each appointment.

What is your perio protocol/program? Advice?

If patients are 4910 you must probe every continued care interval because depths improves in some areas and not in others. To correctly treat the Perio pt you need to know the numbers because the numbers change. You wouldn’t want to clean to the base of a 6mm if it is presently a 4mm. You would ruin the LJE attachment gain etc…. The amount of radiographs seems excessive, but you should be able to squeeze it in in an hour slot. You need to schedule pts according to procedures. If it takes an extra 20 minutes to take pano and bwx then add it on to appt time. Diagnose Perio and tx plan the pts. If you have a good Perio program your production increases and your boss will let you control the time required for certain pts. Run the hygiene side as if you owned it. If you are bringing money in and you are truly focused on the health of the pts and provide the best treatments then you need not be rushed all day long. Discuss necessary restorative procedures and track who schedules from your room. Show the boss evidence of recommended procedures that are scheduled from you.

I update the perio chart once per year and 4 BWs w/ 3 PAs once per year as well.
