Pt has a question about her gums

My pt has a question about an area on her gums. Pt profile is 35yo Pacific Islander woman. The buccal of #29-30 become irritated, itchy if touched, painful upon brushing. It happens before her period cycle 2-3 times a year and lasts about 5 days and has been present for 2-3 years. Upon examination there is a slight composite overhang #29D (symptoms began before restorative was complete #29), no tarter or plaque present, light bleeding, and the gums are firm and pink, no sign of inflammation. #30 has an all ceramic onlay that was placed 8 years ago. Lesions present as small 1x1mm multiple pin point, almost ulcerative-like. I took a photo of the area with my iPhone, is there any way I can attach the photo? My Dr doesn’t seems to be at a stand still as well. Thoughts??

Ulcer-like lesions can be caused by problems related to medications, diseases and malignancies, due to trauma, or have a bacterial or fungal cause. Hormones can play a role in gingival issues too. Because their are many different things this could be, it may be best to refer the patient to an oral surgeon for a definitive diagnosis.
