I would say it’s up to the patient and the specialist. If you are looking for a cookie cutter blanket answer….sorry. If the teeth are so mobile and have extreme bone loss, furcations, recession etc…. Discuss different scenarios and refer to periodontist. Periodontist will give a prognosis for each tooth dependent on what patient wants. If pt wants to keep as many teeth as possible then flap surgeries could be an option. If the pt doesn’t want to save any teeth then implant dentures are an option. If pt doesn’t want implants then dentures are option…..so on and so forth. Pt could want to go back to general dentist and complete S/RP and hold on to all their teeth until one falls out on its own. Etc…
I would say its more about how motivated the patient is and is willing to take care of it! I’ve seen teeth with tons of bone loss last for a long time when taken care of properly. Some people become really motivated when they find out the cost of replacing one tooth! Other would rather just extract than spending the money to replace it. You just have to explain all the options and let the patient decide.
